St. John’s Community Health (SJCH) is a mission-driven Federally Qualified Health Center (FQHC) dedicated to improving community health and reducing health disparities through high-quality, comprehensive care and advocacy. For more than 60 years, we have served historically underserved communities across Los Angeles County and, more recently, Riverside and San Bernardino Counties.
Today, SJCH operates a network of community health centers, school-based health centers, and mobile clinics that provide integrated, whole-person care to patients of all ages. Our services include primary care, family medicine, women's health, pediatrics, dental care, behavioral health, substance use treatment, HIV and Hepatitis C services, transgender health, homeless healthcare, and specialty care programs.
What makes SJCH unique is our commitment to addressing both medical and social determinants of health. In addition to healthcare services, we offer case management, legal services, outreach and advocacy programs, homelessness services, re-entry support, health education, and other wrap-around programs designed to help individuals and families thrive.
